Helping people with Cancer & Dementia/Alzheimer’s
Congratulations to SAP for winning the North Lincolnshire award for services provided to patients living with Long Term Conditions.
Our dedicated Care Co-Ordinator Lisa Dobson has been working hard with Dr Jaafar to help assist patients to make things easier.
These patients can contact Lisa direct who can help get any answers or queries they make have with direct contact to a GP.
Along with trying to make the lives easier of patients living with these conditions, Lisa has also become involved in the Steering Group in Epworth working with the Friends with Dementia group.
The group is aimed at helping make Epworth a Dementia Friendly Town. Along with proving services for people living with Alzheimer’s/Dementia.

The Steering Group have helped set up a Just Sing choir which meet on the 1st & 3rd Thursday of each month at the Thurlow in Epworth. This brings together people, both living with Dementia and their Carers to get together for an hour and enjoy some fun. Anyone is welcome to come and join in the fun!! Refreshments are also available afterwards.
Alongside this, the Steering Group have worked with Dementia Direct at Brigg who provide a service for Carers helping people live with Dementia/Alzheimer’s. The Dementia Direct Service have set up a Memory Café at The Thurlow which is on the 2nd Thursday of each month. Refreshments and activities are available at the Cafe.
The Practice are also working Friends with Dementia and Dementia Direct to provide sessions to help people living with Dementia/Alzheimer’s. Lisa will contact people who have shown an interest in these sessions as soon as dates are confirmed.
