Documentation of proof of address, current within the last 3 months, as well as photographic evidence is required.
All patients are asked to attend a health check with the Healthcare Assistant/Nurse (and to also see a doctor if they are on regular medication). Please inform the receptionist if you wish to register with a specific doctor.
To save time you can also download and print a Registration Form, New Patient Questionnaire and a Summary Care Record Information & Consent Form. You will need to complete these by hand and bring them to the surgery.
You will have a named, accountable doctor who is responsible for coordinating your care. You can still talk to or make appointments to see any of our doctors or nurses, not just your named GP.
